HOW DO YOU SIGN UP?
To sign on as an official Chemo Duck hospital, you simply complete the hospital sign-up form included in the hospital program information packet. Someone from our office will contact you and set up a time to interview you about your desire to be a part of Gabe's Chemo Duck program. Once everything is set up, your first order will be processed.
IS THERE A FEE?
We are committed to providing all children with cancer a Chemo Duck free of charge, therefore we need generous donations from individuals and businesses to allow this to happen. For our hospital program, we offer several funding opportunities. A donation of any amount can be made directly from the hospital, or some hospitals have paired up Gabe's Chemo Duck program with a particular donor. For example, your hospital may have a donor that has a special interest in pediatric cancer. Gabe's Chemo Duck program could be a perfect match for this donor's intentions.
CAN I STILL OBTAIN CHEMO DUCK IF I DON'T HAVE FUNDING?
Gabe's My Heart is able to help match donors to hospitals, so if you are unable to find funding for your hospital, you should still complete the form and your hospital will be matched with an appropriate donor as soon as possible. If your hospital has a donor that has a special interest in pediatric cancer and would like to sponsor ducks for your hospital, please have them contact us directly. Please note that pricing may be different for these donors due to the additional benefits sponsors receive.
DOES GABE'S MY HEART ACCEPT PURCHASE ORDERS?
Gabe's My Heart can provide a W9 number upon request and we can send a donation request to your hospital prior to, or after, you receive your Chemo Ducks.
CAN A HOSPITAL OR DONOR DONATE BY CREDIT CARD?
Donations can be accepted in any amount through our website. Please visit the How To Help page on our website to contact us and to make your donation.
HOW DO WE MANAGE THE PROGRAM?
Typically, hospitals that offer Chemo Ducks to their patients work closely with their child life or social work departments. Since child life specialists and social workers are so closely connected to the patients and families, it makes sense for them to coordinate when to present the patient with their duck. The Gabe's Chemo Duck team is happy to assist you in setting up the program and can partner you with other peer organizations to help discuss their experiences with the program. Usually, hospitals place reorders about once a year. Orders are placed by completing our Chemo Duck order form and returning it to our office. Shipments usually take approximately 12 to 15 weeks.
HOW MANY DUCKS WILL WE BE PROVIDED?
The minimum order of Chemo Ducks is 12 at $25 each, and mutiples of six thereafter. Whether you need 12 or 1000 ducks, we are committed to ensuring the Gabe's Chemo Duck Program is a benefit for patients and families, as well as the hospital.
CAN THIS PROGRAM BE FUNDED BY GRANTS?
Absolutely! We have witnessed many organizations provide Chemo Ducks to their patients by way of grants from businesses and foundations. If you are applying for a grant and need assistance, just let us know. We are here to help.
DOES THE PROGRAM COME WITH MATERIAL?
When a hospital joins Gabe's Chemo Duck program they receive the following:
- One training binder with useful information about Chemo Duck, delivery information and parental information
- Three copies of the Chemo Duck DVD that shows parents how to use Chemo Duck
- Port and Brovaic Ducks, each one accompanied by an introductory book, "Meet Chemo Duck"
- Copies of "I'm Still Me" activity book for use with families
- Access to print all of our printable materials including parent to parent guide
HOW DO I RE-ORDER?
Usually, a facility reorders Chemo Ducks about once a year. Orders are placed by completing our Chemo Duck order form and returning it to our office.
ADDITIONAL INFORMATION
- Once Chemo Ducks are delivered to your facility, they should be distributed by hospital staff with the accompanying program materials listed above. A basic education should also be provided to assist parents in learning how to best use Chemo Duck.
- Please note that once you receive your Chemo Ducks, we ask that they are not returned to sponsors or moved to another facility without permission from Gabe's my Heart.
